General Responsibilities

The Program Nurse is responsible for the direct coordination of healthcare services aimed at meeting the needs of individuals served by the agency.  She/he will participate as an integral part of the Interdisciplinary team.

Essential Duties

  1. Utilizes the nursing process to promote optimal health of individuals served.
  2. Develops and implements healthcare goals based on needs.
  3. Maintains electronic health record for assigned individuals.
  4. Prepares nursing 90-day assessment of individuals in Older Adult program.
  5. Communicates pertinent medical information to appropriate persons, including the program coordinator, families, and day program staff.
  6. Monitors the hygiene of individuals served and initiates corrective action as needed.
  7. Attends specialty medical appointments as needed.
  8. Provides health-related trainings to agency staff and members of the interdisciplinary team.
  9. Ensures completion of medical appointments.
  10. Completes pre- and post-appointment paperwork.
  11. Obtains and transcribes physician orders.
  12. Monitors medication distribution, storage and disposal.
  13. Coordinates ordering of medical equipment and supplies.
  14. Assumes responsibility and accountability for own clinical judgment and actions.
  15. Takes an active role in identifying ethical issues that impact care and advocates resolution.
  16. Assists in identifying and obtaining appropriate medical care.
  17. Provides coverage for nurse colleagues as needed.
  18. Participates in the nursing on-call and clinic rotation.
  19. Educates and mentors new colleagues to job tasks.

Other Duties

  1. Participates on agency committees as assigned.
  2. Performs other work-related duties as assigned.

Basic Qualifications

Education/Training:   Current PA Nursing License, RN or LPN.  Diploma in Nursing, or Associate's Degree, or Bachelor's Degree in Nursing or related field preferred.

Skill(s): Ability to read and write in English, moderate mathematics skills required; proficient interpersonal relationship and communication skills; auditory and visual skills; satisfactory completion of  pre-employment physical exam and drug test as required. Completion and clearance of a criminal background check and child abuse clearance. Has valid driver's license with a clear three-year driving record.

Physical Requirements: Ability to perform the physical duties required of Direct Care Worker. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in providing various care; and operate a vehicle. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds, including transferring of client.

Experience:    Two years experience in an acute or long-term care facility, or working with Individuals who have Intellectual/Developmental Disabilities.
